Waiver/Registration Form
Register by mail with the
registration/waiver* form.
Make sure form is filled in completely,
the WAIVER completed/signed, and a check made payable to "The Dance Workshop" is attached.
Phone Request & Registrations without payment will not be accepted.
If there is a question or a conflict with a desired class, day or time,
please call the studio office at
(708) 226-5658, an alternate class will be suggested;
also new classes will be added to quickly accommodate all students on waiting lists.
An annual $15.00, non-refundable, family registration fee is required.
Only one fee is paid per family, per year.
(Copy of Birth Certificate required for all Preschool & Kindergarten registrations.)
If more than one student is registering, please photocopy blank for for additional student(s).
This year's Summer Dance Session is a five-week/ten
lesson program.
Students are required to attend their choice of classes
twice a week
for five weeks beginning Monday, June 26 and ending Tuesday,
August 1st.
Classes are slotted for either
Monday/Wednesday
OR Tuesday/Thursday.
(Monday, July 3rd & Tuesday
July 4th holidays will be made up Monday, July 31 &
Tuesday, August 1, 2006)
